In the outside play space along the bottom of the door is a metal piece that is sticking out with a sharp edge. Also in the outside play space on the brick wall was a metal grate that had the bottom left corner sticking out with a sharp edge. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Protective Outlet Covers 5 yrs. or less (3270.65)

Protective outlet covers were missing in room 140, 141, and the gym where children 5 years of age receive care. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.
