The Individual Health Care Plan (IHCP) for a child with a chronic medical condition did not include the physician's name or telephone number. Corrective action: A new form with the physician's name and telephone number has been filed.. On the day of the visit Educators didn’t know which children they were responsible for and did not know how many children were in the gym or in the cafeteria. Corrective action: All staff were systematically retrained on the importance of maintaining an accurate attendance list. Unannounced drills have been instituted to catch non-compliance and highlight the importance of the task. Since the issue was that staff responsible for the attendance were slow to record changes because of attempts to multitask, a new policy mandating the responsibility be 1st priority was adopted.. Regulation: daily attendance records indicating each child’s attendance, including arrival and departure times;, The licensee must maintain sufficient numbers of qualified staff to promote the health, safety, growth and development of each child. 7.04(18)(d)(01) — Administration

Through Review of staff records checklist EEC Licensor learned 6 Educators hired in 2024 did not have evidence they competed EEC essenatil trainings prior to working with childrne. Two group leaders do not have evidence on file of work verification for their positions. Corrective action: The educators are in the process of completing EEC essential trainings and will work under supervision until the trainings are completed. The language used to indicate when items are due in employee staff files has been updated. It is now clear that, along with the BRC, the Essentials 2.0 training must be completed before working at the program. The group leaders have resubmitted their forms. 7.04(17)(k) — Administration

The program is keeping electronic documentation of staff meetings. On the day of the visit the Administrator was able to provide electronic evidence of staff meetings for September and October of 2023. There was no evidence for staff meetings in 2024. Corrective action: Staff meeting documentation has been moved to a central electronic location for easy access by phone. Some meetings were saved to a hard drive that was not onsite at the time of inspection.. Regulation: Staff Meetings. The program is not conducting parent conference or offering parent conferences as required by regulations. Corrective action: There is now a plan in place to schedule conferences in January and February, in general. The program did not have a current Building Inspection Certificate on the day of the visit. Corrective action: The facilities director of the school says that the inspection has been completed but we are waiting for the certificate to be disseminated. The inspection certificate will be uploaded to the document library and our licensor will be notified as soon as we we get it.. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR).
