A random review of staff records revealed that documentation of MMR immunization was missing from one staff file. Corrective action: Staff is awaiting answer from physician's office to submit MMR immunization documentation. A random review of children's files showed that not all required yearly updates had been completed. Some files were missing medical records, lead test results, progress reports. Additionally, some IHCPs and medication consent forms were missing required signatures, and some forms were expired. 7.07(01), 7.11(07)(h) — Emergency Preparedness

There was no documentation available at the time of the visit indicating that monthly evacuation drills had been conducted. Corrective action: Documentation is attached. Fire drills occur once a month when school is in session. If the weather is inclement when they are scheduled, we do a "shelter in place" drill. Records for the drills are located in the office on the shelf which is right next to the office door, and kept in a blue binder which is labelled "Evacuation and Fire Inspections".. Regulation: The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The program did not have all the necessary medications available for the children who require them. Some of the medications were expired. Corrective action: new medication has been submitted to classroom.. Regulation: All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer’s packaging., Unless otherwise specified in a child’s individual health care plan, the educator must store all medications out of the reach of children and under proper conditions for sanitation, preservation, security and safety during the time the children are in care and during the transportation of children.
