Review of children’s records revealed that some files were missing currently physicals, enrollment forms, first aid, and emergency medical authorization. In addition, some progress reports and transportation plans, were not dated. Corrective action: Parents were notified about missing documents from children's files. 7.10(08)(b)(02), 7.09(05) — Care of Children

The program did not have a current Staff schedule posted on the day of the visit. Corrective action: Staff schedule has now been posted on the bulletin board in entryway.. Regulation: the Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors., All educators must possess the qualifications required for their positions in accordance with EEC regulations. 7.07(02), 7.07(03) — Physical Facility - Indoors

The posted Building and Fire inspections were expired on the day of the visit. Corrective action: Fire Inspection certificate and Building Inspection Certificates were both printed and posted on the wall. Both certificates were also uploaded to the Documents Library.. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). 7.04(14)(c) — Postings

On the day of the visit the allergy posting did not have a way to protecting children’s medical information. Corrective action: Allergy postings have been written cardstock that is covered, protecting the medical information.. 7.11(01) — Administration

On the day of the visit EEC Licensor was not able to establish that all Educators have completed the required EEC Essentials 2.0. Corrective action: All educators have now completed the EEC Essentials 2.0.. Regulation: Training. All educators must be trained in the program’s emergency and evacuation procedures, in standard precautions and in medication administration procedures. Cited during: Monitoring Visit. 7.11(07)(h) — Emergency Preparedness

The Evacuation drill log did not include documentation of dills for December 102, February or March 2025. The Program did not have evidence of drills conducted once per month as required. Corrective action: Lead Teacher will take over the task of documenting our monthly fire drills starting with April 2025. The drills will be put in a remider calendar for preschool tasks.. Regulation: The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. 7.11(02)(j), 7.11(02)(l) — Health

The program did not have written parental consent to administer medication to a child that was required by an IHCP Corrective action: Director did not realize that the signed form in the child's folder gave consent for the medical diagnosis from allergist but did not include a space for an additional signature by parent for medication administration. The Director downloaded a separate EEC parental consent to administer medication form. The parent read and signed the form and it was added to her child's folder..
