Provider to repair cracked section on fence and riding plastic bike. Correction: Provider corrected one of two deficiency (removed plastic bike from child care space) before the end of the visit. Provider repaired the cracked fence; verified via pictured email.

HAR §17-891.2-41 — Health record on each child

Provider to obtain completed physical exam for one child as discussed. Correction: Worker verified completed physical exam for the child.

HAR §17-891.2-68 — No exposure to electrical outlets

Provider to cover outlets in the bathroom. Correction: Provider covered the outlets during the licensing visit.

HAR §17-891.2-90 — Written practices and policies reviewed with staff, volunteers, and/or caregivers upon hire, start of care, or start of residence and on an annual basis

Provider to review safe sleep policy with household members and one parent of enrolled child and submit verification to worker. Correction: Worker reviewed signed document by parent of enrolled child and household members.

HAR §17-891.2-66 — Written log of practice drills for various emergency situations maintained

Provider to submit log of other emergency preparedness drills conducted . Correction: Worker verified completed emergency drill log and provider will continue to maintain log.
