HAR §17-892.2-27 — Protective measures under outdoor play equipment

The worker inspected play structures in the playground with padded mats beneath it. However, two small slide structure were placed on dirt which director moved to the grassy area during the time of the visit. Correction: Director moved the slides to the grassy area of the playground during the licensing visit.

HAR §17-892.2-35 — Substitutes shall complete at least 10 hours of ongoing training in at least 2 of the department approved topic areas

Director to have one substitute submit Health and Safety training hours to PATCH. Correction: Per director, staff member changed from a substitute to a volunteer working under 10 hours a week.

HAR §17-892.2-61 — Written evidence of fire warning device or system has been inspected initially and annually and approved by a by fire inspector. Report dated: <date>

Director to contact MFD for an annual fire inspection and submit a completed report. Correction: A current fire inspection completed and report on file.

HAR §17-892.2-63 — Accessible electrical outlets shall be covered with child-resistant outlet covers

Director to cover one electrical outlet. Correction: Director covered the outlet during the licensing visit.

HAR §17-892.1-15 — Protective measures under outdoor play equipment

Director to add soft media under the swing hanging from a tree trunk and to remove the other swing from the other tree as it is too close to the tree trunk and tree root. Worker inspected all other play equipment with grass beneath them. Correction: Worker received pictures via email of the swings. Additional soft media was added to under the swing. The other swing have been removed.

HAR §17-892.1-19 — Written evidence of agreement with private physician, or

Director to submit written agreement with a private physician for health consultation. Correction: Worker received letter from DOH public health nurse as the facility's health consultant.

HAR §17-892.1-21 — Written permission from parent/guardian for child to receive emergency care

Director to submit written permission for emergency care. Correction: Worker reviewed consent form via email.

HAR §17-892.1-22 — First aid kit available at all times

Director to supply school with a complete first aid kit. Correction: Director purchased a new first aid kit. A larger kit has been ordered as well per director. On 06/24/2025, director supplied her school with a larger first aid kit.

HAR §17-892.1-31 — Written plan approved by fire inspector, health consultant or Red Cross

Director to submit fire and other emergency situation drill log. Correction: Worker verified fire and emergency drill log.

HAR §17-892.1-11 — Daily attendance records

Director to submit copy of daily attendance on App. Correction: Worker verified daily attendance records via email.
