HAR §17-892.2-62 — Activities and premises minimizes risk of accidental injury

Director to trim mango tree. Correction: Mango's have been trimmed off. Director will continue to trim the branches as needed and plans to install a shade under the mango tree.

HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

Director to submit completed 16 hours of health and safety training to PATCH for two staff members. Correction: Training hours completed for one staff and worker discussed regulations for substitute staff member.

HAR §17-892.2-40 — Written evidence of agreement or contractual agreement for health consultation (private physician, non-profit health organization, community health consultation service, or health advisory board)

Director to submit a letter of agreement with their private physician. Correction: Worker verified a letter from a private physician dated 12/2/25.

HAR §17-892.2-41 — Health record on each child

Director to obtain completed health records for two children as discussed. Correction: Worker verified health records which are now complete.

HAR §17-892.2-63 — Furniture, equipment and toys shall be sturdily constructed with minimal hazards

Director to level the cement and repair the wood on the play structure in the outdoor space. Correction: Repairs have been made to the cement and play structure.

HAR §17-892.1-31 — Written evidence of fire warning device inspected initially and annually by fire inspector. report dated:

Director to contact MFD for an annual fire inspection and submit a completed report. Fire extinguisher was serviced in June 2024. Correction: Fire inspection report dated 11/25/27 received and on file.

HAR §17-892.1-17 — All caregivers meet qualifications

Director to submit copy of CDA certificate to PATCH for verification of CDA completion. Kihei Baptist has the following qualified staff that have been cleared to work with children: 1 Director 1 Teacher and a substitute teacher 3 Aides and a substitute aide Correction: Worker received copy of CDA certificate.

HAR §17-892.1-20 — Health record on each child

Director to obtain immunization exempt form for one child. Correction: Worker received exemption form for enrolled child.

HAR §17-892.1-33 — Potentially hazardous garbage and trash cans shall be properly covered

Director to cover the trash can in the outdoor space. Correction: Trash can has been replaced with one with a lid (pictured).

HAR §17-892.1-33 — No exposure to lead based paint

Director to repair/paint peeling paint on the wooden playhouse and play table in the outdoor space. Correction: Pictures of work done on playhouse and table confirmed via email.
