HAR §17-892.2-27 — Protective measures under outdoor play equipment

The worker inspected the play structure in playground with sand beneath it. However, director needs to place soft media under the climb dome. Correction: Director placed rubber mats under the climber.

HAR §17-892.2-55 — TB clearance

Director to obtain TB clearance for staff as discussed. Correction: TB assessment for staff received; on file.

HAR §17-892.2-61 — Facility shall complete practice drills for various emergency situations at regular intervals, or whenever a new procedure has been added

Director to submit practice drill log for other emergency situation. Correction: Worker reviewed practice drill log; completed

HAR §17-892.2-61 — Written evidence of fire warning device or system has been inspected initially and annually and approved by a by fire inspector. Report dated: <date>

Director to contact MFD for an annual fire inspection and submit a completed inspection report. Correction: Fire inspection report dated 3/23/26 received; on file.

HAR §17-892.1-17 — All caregivers meet qualifications

Director to obtain CANOES certificate from PATCH for two new staff. Children's Garden has the following qualified staff that have been cleared to work with children: 1 Director/Teacher 3 Teachers 1 - Asst. Teacher 6 - Aides 2 - substitute aides Correction: Application submitted to PATCH registry and CANOES certificate received.

HAR §17-892.1-29 — Physical exam clearance

Director to submit verification of completed medical report form (physical exam) for two new hires. Correction: Completed medical form received for one staff. Per director 's email on 01/29/25, the other new staff will work under 10 hours until physical exam is completed and medical form submitted to the department.

HAR §17-892.1-32 — Activities and premises minimizes risk of accidental injury

Director to repair chain link fence and wooden steps (splinters) to the play structure in the outdoor space. Correction: Worker conducted a visual check. Chain link fence has been repaired and wooden play structure has been resurfaced and painted.
