Director to keep Tylenol out of reach of children. Correction: Director removed the Tylenol and placed it in director's office.

Director to cover trash can in one classroom. Correction: Director replaced trash can with new lids.

HAR §17-892.2-63 — Sharp objects, household kitchen items, and tools kept out of reach or supervised when being used

Director to secure adult scissors (keep out of reach of children) in two classrooms. Correction: Director removed the scissors; placed in locked storage and in kitchen.

HAR §17-892.2-63 — Furniture, equipment and toys shall be sturdily constructed with minimal hazards

Director to repair the plastic tricycles (cracked with sharp edges) or remove from outdoor space. Correction: Director removed tricycles and placed them in the back of the storage away from the child care space.

HAR §17-892.1-33 — No exposure to lead based paint

Peeling paint on handles of two tricycles. Correction: Director removed the tricycles from the outdoor space and placed them into the dumpster outside the preschool.

HAR §17-892.1-36 — Sanitation inspection

The facility to provide renewed Food Establishment permit. Correction: Copy of updated Food Establishment permit received; expires 03/19/2026.

HAR §17-892.1-32 — Activities and premises minimizes risk of accidental injury

Adult scissors found in drawer within reach of children. Correction: Staff/Director removed the scissors from drawer and placed the scissors out of reach of children (top of shelf).

HAR §17-892.1-9 — Admission policies that require sufficient information and instruction to be received from parents/guardians that enable caregiver to care for the child

Director to obtain written consent from parents regarding permission for children to have contact with the pet turtle in the classroom. Correction: Pet consent form verified; form on file.

HAR §17-892.1-17 — All caregivers meet qualifications

Director to update the HIEC registry for two staff as discussed. This preschool has the following qualified staff that have been cleared to work with children: 1 - Director/Teacher, 3 - Teachers, 4 - Aides, 2 - substitute teachers, 2 -substitute aides Correction: Worker verified the HIEC registry and staff qualifications have been updated.
