The surface of the diaper changing area was not impermeable. FDCH/LFCCH Handbook, Section 7.18 Comments: During the previous inspection, the Licensing Specialist observed the diaper changing mat was ripped exposing the foam. During the reinspection, the Licensing Specialist observed the diaper changing mat was ripped exposing the foam. The Operator has been given until 1/12/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 7.18: Diapering Area Cleanliness/Sanitization: When children in diapers are in care, there shall be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use. A. The diaper changing area shall be located separate from the food preparation, food service and feeding area. B. Child(ren) did not have a Student Health Examination/DH (Form 3040), or an equivalent health statement on file within 30 days of enrollment. FDCH/LFCCH Handbook, Section 8.2, A Comments: During the time of the inspection, the Operator advised the Licensing Specialist that there are currently 6 children enrolled. During the time of the inspection, the Licensing Specialist monitored 6 of the 6 childrens records. The Licensing Specialist observed one children's file which had an expired immunization form. The Operator was given until 11/16/25, to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 8.2, A: Student Health Records: A. The operator is responsible for obtaining for each child in care a current, complete and properly executed DH 3040, Student Health Examination form, incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, or a signed statement by an authorized professional that indicates the results of the components of the form are included in the health examination from the custodial parent or legal guardian, within 30 days of enrollment. The home did not maintain documentation that the parent(s) or legal guardian(s) of each child were provided information detailing the causes, symptoms, and transmission of the influenza virus annually during the months of August through September in that [The forms were signed outside of the August/September timeframe. ]. FDCH/LFCCH Handbook, Section 8.3, E Comments: During the time of inspection, the Licensing Specialist observed that 4 of the 4 children records contained signed Flu/Influenza and Distracted Adults forms but were dated outside of August and September and April and September timeframes. CCF Handbook Section 7.3, C, 4 and 5, Enrollment Information C. There shall be signed statements from the custodial parents or legal guardian that the childcare facility has provided them with the following information: 1. The Departments childcare facility brochure, CF/PI 175-24, Know Your Child Care Facility, which is incorporated by reference in 65C- 22.001(7)(m), F.A.C. This brochure may be obtained from the Departments website at www.myflfamilies.com/childcare. Local licensing agencies may use an equivalent brochure approved by the Department. 2. The childcare facilitys written disciplinary and expulsion policies. 3. The childcare facilitys food and nutrition policies that includes language on food safety and food allergens. 4. Annually, during the months of August and September, the childcare facility must provide parents/guardians of children enrolled in the facility with information detailing the causes, symptoms, and transmission of the influenza virus. To assist providers, the Department developed a brochure, CF/PI 175-70, Influenza Virus, Guide to Parents, which is incorporated by reference in 65C-22.001(7)(n), F.A.C. and may be obtained from the Departments website at www.myflfamilies.com/childcare. 5.
