Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C Comments: During the inspection, the provider stated that the teacher helped him stand up by pulling his arm. The child has a bruise from the teacher pulling his arm. The director brought this standard back in compliance at the time of inspection by terminating the teacher. An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Comments: During the inspection, Licensing Specialist observed in older toddlers classroom the paint is peeling on the wall by circle time and by the sensory table. In Early Preschool 1 the paint is peeling on the wall by housekeeping and by the shape rug. In Early Preschool 2 the paint is peeling on the green trim in the middle of the wall by the cots, by the left side of the sink and by the shape rug. In Pathways the paint is peeling on the wall by the math area, the left side of the couch and by housekeeping. In Preschool 1 the paint is peeling on the wall by housekeeping. In Preschool 2 the paint is peeling on the wall by the ABC rug and the drywall is peeling and exposing the metal piece the corner of the wall by the sink and the cots. In Pre-K 1 the paint is peeling on the wall by housekeeping, by the library and by the right side of the ABC rug. Also PRE-K 1 room the road rug in the block area is ripped and shredding. The director was given until February 12, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 A.- General Health and Safety Requirements. A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. Child care personnel must ensure sun safety for themselves and children under their supervision. It is recommended that infants younger than six months of age are kept out of direct sunlight, limiting sun exposure when ultraviolet rays are strongest. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 Comments: During the time of the inspection, the Operator advised the Licensing Specialist that there are currently 166 children enrolled. During the time of the inspection, the Licensing Specialist monitored 48 of the 166 childrens records (Letters A through H). Licensing Specialist observed three children who had an expired immunization form. The director was given until February 12, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.1 F Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. F. The facility did not have documentation of a fire extinguisher being properly maintained to include being serviced and retagged timely, and/or with a current certificate. The Director did contact company and scheduled fire extinguishers to be serviced and tagged for Wednesday, 8/6/2025. CCF Handbook Section 3.8.2, B Fire Safety B. Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times.
