FDCH/LFCCH Handbook, Section 4.3, H Comments: During the inspection, the Licensing Specialist observed that the Family Day Care Homes Clearinghouse Employee/Contractor Roster was current. During the inspection, the Operator was not able to provide the Licensing Specialist with the Child Abuse and Neglect form for the Substitute. The Operator has been given until 8/14/2025 to come into compliance with this standard. FDCH/LFCCH Handbook, Section 7.21, A Comments: During the inspection, the Operator had documentation of the last fire drill being conducted/documented on 5/14/2025, with 2 adults and 5 children present. *****Reminder to the provider to complete and document one lockdown drill and one inclement weather drill prior to the expiration of their current licensing year. ******** During the inspection, the Licensing Specialist observed that the fire extinguisher was not conducted annually in that the last inspection was conducted on 5/14/2025. This standard has been completed at the time of the inspection in that the Operator conducted a Fire Drill with the Licensing Specialist during the inspection. There was 1 adult and 3 children present. FDCH/LFCCH Handbook, Section 8.2, A Comments: During the time of the inspection, the Operator advised the Licensing Specialist that there are currently 3 children enrolled. During the time of the inspection, the Licensing Specialist monitored 3 of the 3 childrens records. Licensing Specialist observed one child's file had a missing physical form. This standard has been brought back into compliance in that the Operator provided the Licensing Specialist with an update physical on 7/15/2025. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 7.2 C Student Health Records: The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program.
