Operator failed to maintain a current Employee/Contractor Roster for substitutes, employees or household members in the Clearinghouse. FDCH/LFCCH Handbook, Section 4.1, L Large Family [SR] Comments: During the inspection, the Licensing Specialist observed that the Family Day Care Homes Clearinghouse Employee/Contractor Roster was not current.The provider was given until March 6, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 4.1, L Initial Screening L. The employer/owner/operator must add substitutes, employees and household members to their Employee/Contractor Roster within five days of when the individual has received a child care eligible result. Employer/owner/operator must add an end date within five days of termination for individuals on the Employee/Contractor Roster in the Clearinghouse when employment terminates, or a household member is no longer residing in the home. The home used electronic media time with children younger than 2 years of age. FDCH/LFCCH Handbook, Section 2.5, D Large Family [SR] Comments: During the time of inspection, Licensing Specialist observed two children under the age of two-year-old, they were both one-year-old, watching television. The Owner brought this standard back in compliance at the time of inspection by changing the television to where the children could hear the television but could not see it. During the inspection, the Licensing Specialist provided technical assistance. FDCH/LFCCH Handbook, Section 2.5, D Do not include the use of electronic media for children under two years of age. Electronic media may only be used for educational purposes or physical activity for children 2 years of age and older for no more than 1 to 2 hours per day. A health and safety deficiency having a low potential for harm to the children in care was observed. FDCH/LFCCH Handbook, Section 7.5, B Health Requirements [SR] Comments: During the time of inspection, the Licensing Specialist observed a laptop computer charging cord was accessible the children. The Owner brought this standard back in compliance at the time of inspection by removing the laptop cord out of reach of children. During the inspection, the Licensing Specialist provided technical assistance. FDCH/LFCCH Handbook, Section 7.5, C Strings and cords long enough to encircle a childs neck, such as those on toys and window coverings, shall not be accessible to children. Outdoor play areas were not free from litter, nails, glass, and other hazards. FDCH/LFCCH Handbook, Section 7.6, A Health Requirements [SR] Comments: During the time of inspection, the Licensing Specialist observed on the Little Tikes jungle gym, there were plastic pieces (the stepping stones) that were sharp, long plank laying against the bottom of the fence on the left-hand side of the playground (when facing the playground). The long plank had a sharp end and a nail sticking out, exposing the sharp portion of it, the fence that is connected to the house on the left-hand side of the playground had the ends of nails sticking out (when facing the playground), near the sliding door, the ground cover was exposed and shredding. The Owner brought this standard back in compliance at the time of inspection by removing the sharp plastic pieces from the Little Tikes jungle gym, replaced the long plank with a new plank, added mulch to playground covering the ground cover that was exposed, and nailed in the nails. During the inspection, Licensing Counselor provided technical assistance, 7.6 Outdoor Time, Fencing and Play Area Requirements A. The ground cover or other protective surface under the [Under the swings and by the slide was not maintained.] was not maintained. FDCH/LFCCH Handbook, Section 7.6, K Health Requirements [SR] Comments: During the time of the inspection the Licensing Specialist observed under the swing and by the slide, there was not enough, the Licensing Specialist could see the ground. The Owner brought this standard back in compliance at the time of inspection by adding mulch to the fall zone area. During the time of the inspection, the Licensing Specialist provided technical assistance. FDCH/LFCCH Handbook, Section 7.6, K: Permanent or stationary playground equipment must have a ground cover or other protective surface under the equipment that provides resilience and is maintained to reduce the incidence of injuries to children in the event of falls. Equipment used for climbing must not be placed over, or immediately next to hard surfaces not intended for use as surfacing for climbing equipment. All permanent/anchored playground equipment must be placed over a shock absorbing material that is either the unitary or the loose fill type extending beyond the perimeter of the stationary equipment. Not Applicable Comments During the inspection, the Owner advised the Licensing Specialist that there is no swimming pool on site, therefore this is being marked as not applicable.
