LS gave technical assistance to the director to please check for water leaks and have the stained tiles replaced by the due date. 13.    Child care personnel failed to possess a current attendance record and parent contact information during a fire drill, emergency preparedness drill or an actual emergency. CCF Handbook Section 3.8.4, C and 3.8.5, A (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: During this inspection visit, a fire drill was conducted in the presence of the Licensing Specialist. There were 15 adults and 3 children present. The last inclement weather drill was conducted on 7/23/2025 and the last Lockdown drill was conducted on 9/17/2025. The last approved fire inspection for this facility was dated 10/6/2025. The fire extinguisher certifications are valid until 5/2026. The two and three year old teacher failed to have with her a current attendance record and parent contact information during a fire drill. LS gave TA to the director and staff responsible. This was corrected at time of inspection. FOOD AND NUTRITION 24.    Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C Record Keeping [SR] Comments: Licensing Specialist observed that there is no documentation of background screening results for the staff member listed on the supplemental page. Please ensure to complete all parts of screening requirement by the due date given. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. A copy of each request made to out of state child abuse and neglect registries for individuals who lived outside the state of Florida in the preceding five years. A copy of each search conducted for out of state sexual offender/predator registries for individuals who lived outside the state of Florida in the preceding five years. The individual listed on the supplemental page was missing background screening results according to the director the screening still pending an agency review. Licensing Specialist notified the director that the staff member cannot be in contact with the children until the eligible results from the Clearinghouse are back. The Licensing Specialist (LS) provided Technical Assistance (TA) to the director explaining that this form has to be signed annually . During the time of inspection, the director advised the Licensing Specialist that there 5 employees. During the time of the inspection, the Licensing Specialist monitored all 5 employee records. CCF Handbook, Section 5.1, K Record Keeping [SR] Comments: The facility is responsible for adding child care personnel to their Employee Roster in the Clearinghouse within 5 days of when the individual has received a child care eligible result and has been hired at the facility. The facility is also, responsible for adding an end date for individuals on the Employee Roster in the Clearinghouse within 5 days of the employment termination. The Individuals listed on the supplemental page need to be either added or removed from the Employee Roster in the Clearinghouse.
