Child(ren) did not have a Florida Certification of Immunization (DH Form 680) or a Religious Exemption from Immunization (DH Form 681), on file within 30 days of enrollment. CCF Handbook, Section 7.1, B Comments: At the time of the inspection, the licensing specialist observed 1 childs file had an expired immunization record. The licensing specialist explained the importance of having the correct immunization form to the director and the director understood. Provided technical assistance from the handbook Section 7.1 B, If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age- appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. Technical assistance was given to the director for the painters to stop painting while the children are present, and it was explained that this is a hazard to the children's health and should only be done when the children are not at the facility. One area is a small ramp that is in front of a step up area and it appears to not be connected creating a hazard. The other area is a gap in the flooring that may lead to tripping. The incident report was written out on March 6, 2025, however, it was not signed by the director, the teacher or the mother until March 7, 2025.
