6.4(A), (B) and (D), Handbook: All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. Documentation of an accident/incident was not provided on the day it occurred. Childrens Records [SR] Comments: Complaint alleges that on June 15th 2026, the child was picked up the child from school @ 5 pm. When the parent gave the child a bath she saw a bite mark on the childs stomach. The child stated that a child bit her, and it hurt. The parent was very upset. She went to the school, and the teacher, Ms P., told her there were many students on the day this took place, and it was only her caring for the kids. A complaint inspection was conducted on 6/17/2026 and a statement from the teacher was received. Per the teacher Ms. A.D., the children were playing in the red bus in the playground after 4pm. D.G. and child B were fighting to get on the bus, D.G. sat on the bus first so the other child bent down and bit her on the tummy. When the teacher lifted her clothes, she noticed a little mark. She moved the biter to another area and sat with him for a little while to keep him from going back to D.G. and biting her again. She didnt notify the parent, nor the office about the incident because it slipped her mind and she had 10 children to watch on the playground. On the next day, the parent was irate trying to get into the classroom to see who the biter was. The play area was observed and only one little tikes car was observed, which is the one the children were fighting for. TA was provided on adding more little tikes cars as there are ten 2y/o children that use the area and only 1 car. TA was also provided on better communication tools between staff. Sec. 7-33(c)(2), Ordinance: In groups where children of varying ages are combined, the staff-to-children ratio shall be based on the youngest child in the group. In groups where children of varying ages were combined, a staff-to-child ratio of [1] staff for [8] children was observed. Comments: During a complaint inspection related to diapering of a 1y/o child on 4/15/2026, the school was determined to be out of ratio in the mixed group of 1y/o and 2y/o children in the evening hours after 3PM till 5:00PM. After the 1y/o teacher left the facility at 3pm, the director took the 1y/o children into the 2y/o room to be cared for by the 2y/o teacher and left the premises at 3:30PM. As confirmed by the teacher and director, the teacher was alone with both groups of children. Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [child's soiled diaper not changed]. Ratios/Discipline [SR] Comments: Allegation: The child, M.S.(1y/o), is going home everyday, with a soiled diaper. The same amount diapers the parent brings the child with is them amount of diapers he comes home with. The child has a diaper rash on private area. The child was soaking wet yesterday, the parent told the Director about this and they told him it was on CAMERA she said the director admitted he was not changed. FINDINGS: A complaint inspection was conducted on 4/16/2026 and a statement from the teachers and director was received. Per the lead teacher, she left at 3:00PM on Wednesday, 4/15/2026 and the child was dry when he woke up from nap at 2:00PM so she didnt change his diaper. She left the children in their assigned room with the director at 3pm. Per the director, she didnt stay in the room with the 1y/o children. She took them to the 2y/o room instead and left the premises at 3:30PM. The mom picked up the child at around 5:30PM and called at almost 6PM to say that her child was overly soiled. Per the 2y/o teacher, this is the first time they mix the 1y/o group with her 2y/o group. The 1y/o children usually mix with another 1y/o group. She was alone with 11 children, including the 1y/o children. The office staff went into the classroom, checked diapers and left so she was alone with the group. She didnt get a chance to change M.S. when the mom arrived to pick him up around 5:30PM. The mom stood by the door, called him and he went to her. She left and didnt say anything to the teacher. Per corroborating agency who reviewed video footage from the 1y/o classroom, the child in question was not changed all day. The teacher checked the childs diaper once in the morning. She picked him up, put him on the diaper changing table, checked his diaper and put him right back down. Prior to naptime, she washed the childs hands and face and put him down for a nap. After naptime, the teacher checked his diaper again and did not change him. After reviewing the childrens daily schedule, the teachers and corroborating agency statement, It was determined that the child was not changed in the morning, prior to naptime and was not changed for the remainder of the day. The child was picked up at 5:32PM. This complaint is substantiated and a non-compliance for inadequate supervision is being issued because his needs were not met, in that, he was not changed while being cared for at the facility. The child was on the premises TA was provided on routine diapering and following the diapering schedule. 08. Initially the daycare staff said that they had no idea how she received the injury. The staff then said that the child fell. There were some children who said that there were three six year old children that bullied the child and caused the injury. FINDINGS: A complaint inspection was conducted on 8/29/2025 and a statement from the Director was received. Per the Director, she was unaware of the incident. The mother pointed out the lip injury to the director at pick up time on 8/25/2025, so she wrote a report documenting what the mother stated. The director also said that she had not received any notification from the teacher about the child receiving this injury. Three different incident reports were created for the same incident. The director documented what the mother stated, the teacher documented the incident and a third incident report documented what the child told a therapist at the school. Parent was given the incident report that was written by the Director. This report only stated that the mother discovered the mark on the lip while she was leaving the center. The actual incident report that documented what happened was not given to the parent and the director was not aware of it. TA was provided on accurate incident documentation and providing it to parents at pick up time. This complaint is substantiated. 41. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. Lack of direct supervision was observed in the [incident where child left gained access outside the facility]. Ratios/Discipline [SR] Comments: Complaint alleges that child A was on the street in the middle of the road. The parent reported the situation and the director checked the cameras and said that that the child was trying to get into office and was able to get out when one of the parents opened the door. The office was very busy. FINDINGS: A complaint inspection was conducted on 3/12/2025 and statements from the director and office administrator was received. Per the director, she stated that the incident happened on 2/27/2025. She was tending to a child that was crying in the office a the time of the incident. It happened near closing time when the child was sent to the office with another child. He went through the security door and into the lobby. There was a parent holding the front door open when another parent approached from behind him. He walked out the door in front of the parent that was going out the door. Another child walked Child A up to the office. Video of incident was reviewed and the following was observed; Date stamp on video: 2/27/2025 at 15:19. Camera view is facing the lobby area and main entrance door. A parent in a black flowery dress is observed opening the entrance door and standing at the door holding it open. Child A comes into camera view walking toward the entrance door, wearing a blue bookbag. He stands in front if the open door. There are two more adults that are in camera view, One appears to be signing a book and the other is walking toward the door behind child A. As a parent walks toward the door, child A walks out in front of that parent and out of the building.
