Sec. 7-32, Ordinance: Before the first day of employment at a child care facility, all child care personnel, excluding temporary substitutes, must have on file at the child care facility a signed statement from a licensed physician or authorized agent of the Florida Department of Health attesting that the person is in good health in order to care for children in a child care facility setting. The health assessment statement shall be documented on a form prescribed by the local licensing agency and updated at least every two years. Child Care personnel did not have a current health statement on file. Comments: There were 2 staff members that had an expired #8 physical form. See supplement. 06. 3.1(A), Handbook: All child care facilities must be in good repair, free from health and safety hazards. Areas of the facility were found to be in need of repair in the following areas [see below]. Facility/Storage [SR] Comments: The following areas are in need of repair: There is a broken paper towel dispenser in the shared bathroom that needs to be repaired. 1. The closet door in the 4 year old classroom has a latch lock that needs to be removed. 2. The Alphabet rug in the 4 year old classroom needs to be secured to the floor or replaced. 3. The trashcan lid in the shared bathroom was observed to be rusted. 4. There is a broken soap dispenser n the shared bathroom that needs to be repaired or removed. 5.
