   3.6.4(B), Handbook: When napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician. Written documentation of a physicians authorization must be on file at the facility. Documentation must include the childs name, childs date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid. There was no documentation on file that an alternative position/swaddling was authorized by a physician for an infant in care. (Section 2.1(A) Health and Safety, Page 3) Indoor/Outdoor Space [SR] Comments: Upon arriving to the infant classroom, one infant was observed alone in the crib sleeping with a wedge under the cribs mattress. Monitor asked if there was a written authorization by a physician to allow the child sleep in an alternate position and provider answered no. Provider proceeded to remove the wedge at the time of the inspection to bring this area back into compliance. Technical assistance was provided regarding this standard. 21.    3.9.1(B), Handbook: Child care personnel, while working in the food preparation area, must wear proper head covering, such as a hair net or hat. Child care personnel working in the food preparation area did not wear proper head covering. (Section 2.1(A) Health and Safety, Page 3) Food Service/Nutrition [SR] Comments: Upon arriving to the kitchen area, a staff member was observed serving meals without wearing proper head covering. Staff member proceeded to wear head covering to continue working in the food preparation area. This standard was brought back into compliance at the time of the inspection. Unrelated the above statement, verification of meals temperature was recorded and observed in compliance during todays visit. 34.
