Child care personnel failed to possess a current attendance record and parent contact information during a fire drill, emergency preparedness drill or an actual emergency. License/Certificates [SR] Comments: During the course of a fire drill, attendance records were checked and the following was observed out of compliance; The 3s and 4s group exited the premises through the playground and out to the parking lot. They did not have attendance records with them nor emergency contacts for the children. An area(s) of the facility was observed to not be in good repair. (Section 2.1(A) Health and Safety, Page 3) Facility/Storage [SR] Comments: The following areas of the facility were observed to not be in good repair: - Chipping paint throughout the facility - Clean the stained area rugs (all of the classrooms) - Water stained ceiling tile (toddler's classroom) - Peeling base boards (entry doorway toddlers hallway) - Repair/replace the missing base board (Vpk classrooms) -Secure loose screw on cabinet door knob (white cabinet located in the Vpk classroom) - Torn diaper changing pad (toddler's classroom) - Equipment in the outdoor space was observed to be exhibiting rust (both playground structures) - Repair the loose green gate pole (along side the back fence) - Cut the ingrown weeds and tree branches that's protruding through the fence 11. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. 3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. Indoor/Outdoor Space [SR] Comments: - The two climbing/slide combination structures on the playground have signs of rust. The larger structure is closed off on the top section because the deck structure is compromised. The metal has thinned and may break of continued use without repair. The deck in the lower area of the structure has also been compromised. Although the damage is less, The metal has begun to flake in certain areas, therefore, thinning the structure. The second play structure has rusty handle bars that have thinned and have holes near the ends, connecting to the play structure. - Repair the broken wooden panels on the fence - remove/repair what appears to two steel poles on the ground near the fence with exposed edges -repair /cover the exposed concrete with jagged edges near the right side of the bike shaded area TA was provided that the larger play structure should not be used until the necessary repairs are done Source: FL DCF CARES inspection 2025-01-06.
