40. Accident/Incident Reporting Group: DCF Handbook (40-03)
6.4(A), (B) and (D), Handbook: All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. Documentation of an accident/incident was not provided on the day it occurred. Childrens Records [SR] Comments: Complaint alleges that the child (E.C.) currently has a bump and gash to the forehead. The director and teacher at his school are unable to explain the injury. On two other separate occasions, E.C. has had unexplained injuries. On February 3rd, E.C. sustained a scratch on the left cheek. On February 13th, his lower lip was split and swollen. There is concern expressed about staffs supervision given E.C. has sustained multiple unexplained injuries. FINDINGS: A complaint inspection was conducted on 3/10/2026 and a written summary of events was received from the owner. Per the owners written statement, the assistant teacher Ms.M.P. reported that on 3/5/2026; She did not observe the bump or scratch at any point during the day. The only moment she observed E.C. crying occurred at approximately 4:30 PM. At that time, E.C. and another child in the class were arguing over a toy car. Ms. M.P. stated that she: Separated the children immediately Comforted Enaje, who was crying Attributed the crying to frustration over the toy She further reported that she: Redirected Enaje to another activity Offered him water Observed that he calmed down and engaged in the new activity without further signs of distress Ms. M. P. stated that she did not observe any visible injury at that time. Per owners interview with the lead teacher on date on incident at 6:00 PM Interview with Lead Teacher (Ms. G. ) Ms. G. reported that: E.C. did not have any visible bumps, scratches, or marks on his head prior to her departure at 4:00 PM. She recalled specifically that it was Crazy Hair Day at the school. Before nap time, she removed the hair decorations from E.C.s hair, which required direct contact with the childs head. Video footage was reviewed and the owner pin-pointed when the incident could have happened and offered to show the mother the video footage the following day. At 4:27PM on video, the child was observed fighting for a toy with another child. He fell and gets back up, and walks to the teacher. The teacher grabbed him by the hand and walked with him to the toddler tables to redirect him to another activity. As they walked toward the tables, the child is seen holding the right side of his head. Then, teacher is seen trying to stop the childs cry by offering other activities, the child cried a little longer and at around 4:30PM, is seen interacting with another activity. The school failed to complete an accident/incident report on the date of the incident, as the teacher was unaware that E.C. was hurt. TA was provided on properly assessing the child after a fall/incident and documenting a report to be provided to the parent prior to arrival. 41.
