3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. Comments: -Playground equipment needs to be clean. Stains that appear to be mold were observed on some of the play structure. - The stair on the light green double slide was observed rusted and needs repair. - The 2 poles underneath the double slide at the PreK playground were observed rusted and needs repair. - The wood planks border surrounding the toddler playground rubber surface were observed broken, lifted and damage. The health assessment statement shall be documented on a form prescribed by the local licensing agency and updated at least every two years. Child Care personnel did not have a current health statement on file. Comments: Two staff members did not have a current medical statement on file. 06.
