Sec. 7-44(f), Ordinance: The childs arrival and departure must be recorded by child care personnel at the time the child enters and departs the child care facility. The attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. In order to account for childrens whereabouts, children who are reassigned to different classrooms throughout the day should be signed in and/or out of the rosters for those respective classrooms each time. The facility failed to maintain an accurate attendance roster for the children in care. License/Certificates [SR] Comments: Daily attendance record in two classroom was not accurate at the time of the inspection. Both classrooms did not properly document the time the child enter/departure the classroom. Technical assistance was previously provided on 6/5/2025 for the same standard. This area was corrected at the time of the inspection. 2.5.4(A), Handbook: All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order. Documentation by the mechanic must be maintained in the vehicle. Provider did not obtain inspection by a qualified, certified mechanic and retain documentation in the vehicle. (Section 2.1(A) Health and Safety, Page 3) Transportation [SR] Comments: During todays visit, provider was not able to show proof of a current vehicle inspection. Provider requested a copy from the insurance company, which was submitted to the license monitor via email. This area was corrected the day of the inspection. Technical assistance was provided regarding this standard.The following information is unrelated to the above statement:Vehicle with VIN #3874 was inspected and the following information was obtained:Vehicle insurance is valid until 4/20/2026.Vehicle alarm, seatbelts, first aid kit, fire extinguisher, and vehicle signage were observed in compliance during todays visit. Sec. 7-35(a), Ordinance: Children shall not be permitted to touch animals without adult supervision. Reptiles shall not be handled by children. 3.1(C), Handbook: Animals that are poisonous, venomous, aggressive, or pose a potential threat of harm to children in care are prohibited. No animal may freely roam the indoor/outdoor premises. Animals are prohibited in areas where food is prepared, stored, or served. If animals or birds are kept in classrooms, they shall be caged and cages must be kept clean. This area was out of compliance in that [a pet was on premisses that posed a potential threat of harm to the children]. Facility/Storage [SR] Comments: Complaint alleges that a teacher got bitten by a dog on Friday during pet week. Child is terrified by what he/she says they saw. A dog was inside the school. There is concern about vaccination and allergies that children may have and be exposed to. Parents are not notified or asked for a written consent. FINDINGS: A statement from the director and teacher was received. Per director, it was take your pet to school day. She was the only one who brought her pet to school. She was asked if there was a schedule for the activity and she stated that it was from 10 to 11 but that the dog was in the building all day because she was still at work. The incident happened at around 4:40PM. The teacher grabbed the dog and the dog bit her. She stated that there may have been children in the hallway but doesn't believe that they saw the incident. Per the teacher, she was leaving for the day and there were children lined up in the hallway, against the wall, with the director. The dog was on a leash being held by the director, laying on its back, so she bent down to pet it. While she was petting it, the director asked her to stay with the children while she checked something out in the playground. As the director walked away, she pulled the dog's leash by accident so the dog scratched/bit her near her ear/chin area. She started to bleed and the children got scared. TA was provided on prohibiting animals that pose potential threat of harm to the children in care. This portion of the complaint is substantiated. Some children files were checked for compliance for ages 2, 3, and 4. There is a document on file named "Animal Encounter Authorization" that parents signed upon enrollment and the pet is vaccinated. 2.5.2(A)(1-5), Handbook: A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must include: (1.) Each childs name, (2.) The date and time of departure, (3.) Time of arrival at the destination, (4.) The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and (5.) The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). 2.1(E), Handbook: The child care facility must not be used for any business or purpose unrelated to providing child care that can interfere with compliance with child care standards or permit the unsupervised presence of individuals who do not meet screening and training requirements (with the exception of parents or legal guardians of children in care) when children are present. A business was operated in the children's play space when children were in care. Facility/Storage [SR] Comments: Complaint alleges that a child currently in care at Everglades Academy Preschool, referred to be evaluated at KOAKA ABA in Miami and has falsely been assigned a therapist in house at Everglades Academy in Pembroke Pines. The current director for Everglades Academy is in fact an employee of Koala. A complaint inspection was conducted on 2/11/2025 and a statement from the director was received. Per the director, C.B-M., The director stated that Koala is a company that they refer the children to for therapy services. The director was asked if the business is located on-site to which she responded no. They just refer their families to that company for services but don't require that their children use that company. She was asked about working for the school or the therapy company and she stated that she used to be the owner of Koala ABA and that is the reason for her wearing a shirt with the Koala logo on it. Upon arrival to the facility, there are two different decals on the entrance doors. One door decal says Everglades Academy with the phone number and website, and the other door has a decal that says Koala ABA and Learning Centers with the phone number, website and hours of operation being Monday- Friday 7:0AM to 6PM.The director was observed wearing a shirt with the logo Koala ABA and and Learning Centers. I visited Koalaaba.com and under the locations tab, The preschool address is presented as one of the locations for the business and the director of the preschool is listed as the executive director under the about/our team tab. A statement from the owner could not be received as her phone sounded off and my call could not get through.
