Sec. 7-39(c), Ordinance: At least one permanent bathtub, shower, or deep sink shall be available for bathing children at all times. The facility did not have a tub or shower readily available for emergencies. Comments: The bathtub was not operable at the time of this inspection. The main lever handle to turn on the water was calcified and would not turn. Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [see details below]. Ratios/Discipline [SR] Comments: During naptime, the monitor observed [1] staff personnel watching two partitioned classrooms and an additional classroom space. The monitor brought this concern to the director walking in the building. Once the director returned to the partitioned classroom and rearranged groups to align with ratio requirements, this standard was brought into compliance. 50. Daily Attendance Group: DCF Handbook (50-02)

Sec. 7-44(f), Ordinance: The childs arrival and departure must be recorded by child care personnel at the time the child enters and departs the child care facility. The attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. In order to account for childrens whereabouts, children who are reassigned to different classrooms throughout the day should be signed in and/or out of the rosters for those respective classrooms each time. The facility failed to maintain an accurate attendance roster for the children in care. License/Certificates [SR] Comments: During today's inspection the monitor found inaccurate attendance recordings in the 2's and 3's classroom. The monitor brought this concern to the Director and teachers on site. The teachers immediately corrected the attendance record sheet, bringing this violation into compliance.
