The facility's written plan of scheduled activities was not followed. CCF Handbook, Section 2.6 General Requirements [SR] Comments: On 3/19/2026 licensing counselor took pictures of all classrooms daily schedules. The daily schedule displayed in the classrooms clearly show that the outside times are specific for each classroom to rotate the playground. According to the daily schedules posted the latest time outside would be 4:40 pm for the four year old classroom. The complaint states that the children are outside until 5:00 or 5:30 pm daily. On 3/24/2026 licensing counselor asked each teacher if they follow their specific classroom daily schedule and all teachers said that they do go outside at their specific time but they do stay out later because of ratio dropping and they combine on the playground as children are picked up. Licensing counselor spoke with the director and she stated that they are never outside past 5:00 pm because most children are picked up by then and she will wait with the few children that are remaining inside the facility. The director has made the correction by posting new daily schedules that reflect classrooms being outside until 5:00 pm. CCF Handbook, Section 3.7, I Physical Environment [SR] Comments: On 3/19/2026 licensing counselor checked all of the bathrooms for cleanliness and for good operating condition. All of the bathrooms were in clean condition. One toilet was not in good operating condition. The toilet was missing the toilet tank lid. Licensing counselor took a picture of the toilet. On 3/24/2026 licensing counselor checked all of the bathrooms again and all of the bathrooms were in clean condition. The facility director called the owner and stated that the toilet tank lid had been previously ordered but had yet to arrive. The director assured licensing counselor that the toilet tank lid would be replaced that evening. On 3/26/2026 licensing counselor checked all of the bathrooms again and all of the bathrooms were in clean condition. The facility director was not on site but called and stated that the plumber had ordered the toilet bowl lid and not the toilet tank lid. Therefore, they would have to wait for the correct lid to be delivered. On 4/2/2026 the director sent licensing counselor an invoice from the plumber and a picture of the toilet with the toilet tank lid on it.
