CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the time of the inspection, the Licensing Specialist observed in the infant room kitchenette the half wall has exposed particle board that cannot be properly sanitized. In transportation vehicle 04481: seatbelt plate covers are missing In transportation vehicle 03212: seal belt plate covers and cupholder covers are missing. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. 13.    The play equipment was not maintained in a safe or sanitary condition for the children to use in that [equipment was damaged and rusted]. CCF Handbook, Section 3.12, B and F (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: During the time of the inspection, the Licensing Specialist observed the tunnel structure on the small outdoor play area was crack on the top and repaired with tape. on the big outdoor play area the yellow instrument has frayed metal from where the stick once was. The yellow slide with the green ladder, the coating is peeling away from the grate which is exposing rusted metal. A portion of the grate has broken showing a hole 3 inches in width. The blue paint on the ladder is beginning to chip away exposing rust. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts.    The ground cover or other protective surface under the [equipment] was not maintained. CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: During the time of the inspection, the Licensing Specialist observed several pieces of playground equipment which require fall zones did not have a resilient surface such as mulch which extended 6 feet (measured between 4 and 5 feet) out from the structures and 6 inches (measured between 1 and 3 inches) in depth all around. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. 2. If the ground cover in place is a unitary playground surface, then the unitary surfacing materials must be installed, maintained, or replaced according to manufacturers instructions. Unitary surfaces must be tested to and comply withASTM F1292; documentation of test data must be retained at the facility and available for licensing to review. 3. If the play area was approved by the Department prior to January 1, 2020 and does not meet the 6-foot fall/use zone requirement, then the facility must submit a written notification, including photographs and layout of the play area, to the Department prior to or on April 1, 2020, notifying its intention to continue to operate using the prior approved play area. However, if the permanent or stationary playground equipment is moved or replaced then the standard would apply, and 6 feet of use zone must be provided.
