The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 Comments: During the time of the inspection, the Licensing Specialist observed 1 child had expired immunization records. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A -1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. CCF Handbook, Section 3.1, A Comments: During the time of the inspection, the Licensing Specialist observed several ceiling tiles with stains in the 3 and 4-year old room. The sink in the 2-year old room is in good repair n, the Licensing Specialist provided technical assistance: per the CCF Handbook "A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months." 13. The facility used cribs that did not meet current federal guidelines for construction regulations as outlined in Title 16, Parts 1219,1220 & 1221 Code of Federal Regulations. CCF Handbook, Section 3.6.3 Comments: During the time of the inspection, the Licensing Specialist observed 7 cribs in the infant room. 3 manufactured 8-2011, 2 manufactured 11- 2010, 2 manufactured 2-2023. During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "Children up to one year of age must be in a crib or playpen/play yard with sides for napping. Crib sides must be secured while there is an infant in the crib. Bar spacing must not exceed two and three-eighths inches. Cribs or playpen/play yards must meet the regulations as outlined in Title 16, Parts 1219, 1220 & 1221 Code of Federal Regulations, 2014, which is incorporated by reference in 65C22.001(7)(w), F.A.C." CCF Handbook, Section 3.11 Comments: During the time of the inspection, the Licensing Specialist observed the table in the 2-year old and 1-year old rooms with built in seats to not have the manufacture straps attached. During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "Equipment, materials, furnishings, and play areas should be sturdy, safe, and in good repair. Provider should monitor the Consumer Product Safety Commission (CPSC) recommendations for use of equipment. Walls, ceilings, floors, furnishings, equipment, toys, and other surfaces should be suitable to the location and the users. They should be maintained in good repair, free from visible soil and clean. Equipment and furnishings must be arranged to allow direct supervision of children by child care personnel from all areas within a room/play area. Bookshelves, televisions, and other heavy furnishings must be stable and secured as to not pose a threat of falling over. In efforts to prevent children from climbing to dangerous heights or enabling them to reach hazardous items, chairs and other furnishings must be placed away from cabinets and shelves. Employment history check did not include all required information including applicants, job title, description of regular duties, confirmation of employment dates, and level of job performance. CCF Handbook, Section 5.1, D Comments: During the time of the inspection, the Licensing Specialist observed that 7 personnel had incomplete employment history verifications in that they were missing the employment history check, social security numbers, and employment dates. During the time of the inspection, this was resolved by the Operator filling in the information. During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "D. The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained."
