A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.4 (Section 2.1 Health and Safety, Page 3) By January 1, 2022 all vehicles used by childcare facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed the white shelf in the preschool classroom was broken and had exposed nails. It also had peeling paint. In the toddler classroom, there was a shelf on the right side that had nails sticking up in multiple spots. In the toddler bathroom, there was a toilet marked out of order. In the van there were tears in the floor covering, in the arm rest of the arm rest of the 2nd row seat, and a tear in the back of the 3rd row seat. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 A.- General Health and Safety Requirements. A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. CCF Handbook, Section 3.5, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed on the big playground there is rust under the bridge section. There is a black substance on the sliding boards and tunnel. The wood barrier by the slide, is deteriorating which is exposing a metal spike. The playhouse has a black substance on the inside of the roof and a broken piece of plastic on the back which is sharp. Behind the playground, there is another area where there are broken logs which are sharp. On the toddler playground, there is a black substance on the inside of the playhouse. The wooden house structure has deteriorating wood with sharp edges, exposed broken trim, and roofing nails that show through to the bottom where the children have access. There was an active bees nest under the roof of the wooden structure. The metal climb through toys had rust in them that was flaking and sharp. The blue rocking toy had rust on the steering wheel, and the red rocking toy had rust that was sharp on the handlebars. It was measuring 3 foot 4 inches-3 foot 7 inches. The fence at the back of the preschool playground was measuring less than the minimum 4 feet. From the fence that divides the playground, 5 sections going to the left. Measurements were 3 foot 9 inches-3 foot 11 inches.
