The facility did not have documentation of a fire extinguisher being properly maintained to include being serviced and retagged timely, and/or with a current certificate. CCF Handbook, Section 3.8.2, B Comments: During the inspection, the Licensing Specialist observed the facility did not have documentation that two fire extinguishers were properly maintained to include being serviced and retagged timely, and with a current certificate. The Operator has been given until 7/5/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 3.8.2, B: Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times. 30. Diapering CCF Handbook, Section 3.10 (30-12)

The container used for the disposal of soiled diapers or the storage of used cloth diapers was accessible to children. CCF Handbook, Section 3.10.2, J & K Comments: During the inspection, the Licensing Specialist observed soiled disposable diapers were accessible to children in care in that they were in trash receptacles that could be opened by children. The classrooms that had accessible trash receptacles were Twaddlers A, Twaddlers B, Toddlers A, Toddlers B, and Preppers B. This standard has been brought back into compliance during the inspection, in that a staff member added a child proof lock to all of the trash receptacles containing soiled diapers, making them inaccessible to the children. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 3.10.2, J.: Diapering Requirements: J: Soiled or wet disposable diapers must be disposed of in a plastic lined, securely covered container that is not accessible to the children. The container must be emptied, cleaned and sanitized or disinfected, at least, daily. K. Soiled cloth diapers must be emptied of feces in the toilet, and soiled or wet cloth diapers shall be placed in a securely covered container that is not accessible to the children. 45. Background Screening Documents CCF Handbook, Section 7.4.1 (45-02)

Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C Comments: During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there are 28 employees. During the time of the inspection, the Licensing Specialist reviewed the Clearinghouse roster for 28 of the 28 employees. During the time of inspection, the Licensing Specialist observed 1 childcare personnel was missing documentation of Level 2 Clearinghouse screening clearance that states Childcare Eligible. The personnel member was present during the time of inspection and observed working with children with one other childcare personnel member in the classroom. This standard was resolved at the time of inspection when the Operator contacted the Clearinghouse. The Clearinghouse then gave the provider documentation Level 2 Clearinghouse screening that states Childcare Eligible for the personnel member. The personnel member is now marked as DCF Childcare Eligible in Clearinghouse as of 10/03/2025. CCF Handbook, Section 7.4.1,C A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. REMINDER: Care Provider Background Screening Clearinghouse: The Background Screening Clearinghouse will generate e-mails and send you, the provider, a reminder at 60, 30, and 21 days prior to the retention date expiration. Please note, you will only receive that notification for the staff that are on your facility roster. Please keep your roster current in the Clearinghouse to avoid confusion on expiration dates, or unwanted notifications.
