Background screening was not completed: [ after a 90-day break in service.] CCF Handbook, Section 5.2 Record Keeping [SR] Comments: At the time of inspection, the Licensing specialist determined that the child care personnel listed on the supplemental had a break in service on their existing background screenings previous to starting at the facility. Staff 1 started in the industry on 6/15/2022 and left the industry later that year in October. The staff returned to the facility on 01/2025 and left the facility mid-year in 2025 and was rehired 3/3/2026. The staff had an existing eligibility date in Clearinghouse of 5/24/2022. The specialist advised the provider that the staff had breaks of service in the industry that exceeded 90 days and staff must be resubmitted in background screening. Staff 2 started in the industry on 5/2023. The staff left the industry on 6/2024 then returned to the facility on 10/6/2025. The staff started at the facility on 11/8/2025. The provider initiated a resubmission for both staff on todays date. TA was provided to ensure that the child care personnel who have a break in service of more than 90 days from the industry are resubmitted in the Clearinghouse. An employment history verification was not documented and on file. CCF Handbook, Section 5.1, D Record Keeping [SR] Comments: At the time of inspection, the Licensing specialist observed the previous employment for the child care personnel listed on the supplemental and determined that verification had not been documented previous attempts just noted called seven times. The specialist suggested the provider attempt to call again to the previous child care facility and the provider did and was able to speak to the owner who verified employment at time of inspection. The specialist also determined that the staff had previous work history within the past five years that was not documented nor verified by the provider. The provider immediately called to verify employment for the additional employment at time of inspection and verified all employment. TA was provided to ensure that all previous work history is verified and documented that verification was complete by the due date. Children were provided food that was not age appropriate and/or posed a choking hazard. CCF Handbook, Section 3.9.3, B & C Food and Nutrition [SR] Comments: At the time of inspection, the Licensing specialist observed the children in the one and two year old classrooms in the middle section of the building eating lunch. The children were served chicken nuggets, rice and garbanzos beans, green beans and whole chucks of mixed fruit. The toddler children were observed with whole dinosaur shaped nuggets and whole chucks of mixed fruit to include pineapple and peaches and grapefruit. The specialist advised the provider that the food need to be cut into smaller pieces to eliminate a potential choking hazard. The provider advised the staff to cut the food items into smaller pieces for the children with food remaining at time of inspection. TA was provided to ensure that the food items provided to children in care infants and toddlers are age appropriate to include food size. CCF Handbook, Section 3.5, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: At the time of inspection, Licensing Specialist observed a purple plastic caterpillar crack with sharp edges on the playground that can cause injury to a child. It appeared that the provider attempted to repair the play equipment with a metal clamp which was now stick up cause a potential sharp cutting hazard to children in care. Technical assistance was provided to repair or replace any damage play equipment.
