Child care personnel failed to share the accident/incident form with the custodial parent/ legal guardian or the individual authorized to pick up child on the day that the incident occurred. CCF Handbook, Section 6.4, B & D Comments: Specialist interviewed the teacher that witnessed the incident February 20th, 2025. She was busy and forgot to notify the parent or have the parent sign the accident/incident report when the child was picked up. When the parent came to pick the child on the last day of enrollment, they asked for all accident/incident reports involving their child. That was when it was discovered that the parent was not notified. The parents signed the incident report once it was discovered. The Child Care Facility Handbook states on pages 55 and 56: B. Child care personnel failed to immediately contact emergency services and the child's custodial parents or legal guardians in the event of a serious illness, accident, injury or emergency to their child.
