Bottles and/or sippy cups brought from home were not labeled with childs first and last name. CCF Handbook, Section 3.9.5, B Comments: During the playground inspection, the Licensing Specialist observed several water bottles that did not have the children's names on them. There was also a large spray bottle that contained water that was not labeled as well. The teachers labeled the water bottles during the inspection. The Child Care Facility Handbook states on page 35 that: B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily. 03. Ratio Sufficient CCF Handbook, Section 2.3 (03-01)

A ratio of [one ] child care personnel for [six] children is required. A ratio of [one] child care personnel for [eight] children was observed. s.402.305(4), F.S. Comments: The inspection was conducted during napping hours, the ratio observed was: Four/Five's 11:1 Two/Three/Four's 14:1 (majority of children were age 3). One/Two's 8:2, however, prior to Licensing Specialist entering this room, the second teacher was observed coming back from lunch and was told by the Director to go into this classroom. When the LS reached the classroom there was 2 employees, but when asked, the teachers admitted that there was only one teacher present while the other teacher was at lunch. The LS discussed naptime exceptions with the employees and the Director. The Child Care Facility Handbook states on page 13 that: 2.4.2 Nap time Supervision During nap time, child care personnel must be within sight and hearing of all the children. All other child care personnel required to meet the staff-to-child ratio must be within the building on the same floor and available to be summoned if needed to ensure the safety of the children. 17. Outdoor Play Area/Fencing CCF Handbook, Section 3.5 (17-07)

The base of the fence in the outdoor play area was not at ground level and could allow inside or outside access by children or animals. CCF Handbook, Section 3.5, F Comments: During the inspection, Licensing observed a part of the fence on the large playground that was no longer ground level. The facility will need to repair the fence. The Child Care Facility Handbook states on page 24: E. The facilitys outdoor play area must be fenced as required by local ordinances to prevent access by children to all water hazards within or adjacent to outdoor play areas, such as pools, ditches, retention ponds, and fish ponds. F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. Child care personnels Level 2 documentation was incomplete in that [ A copy of out-of-state child abuse and neglect registry search is missing., A copy of out-of-state sexual offender/predator registry search is missing.]. CCF Handbook, Section 7.4.1, D, E, and F Comments: There are currently 10 employees. All 10 employee's records were reviewed. One employee, SB, that was hired on 8/8/2024, lived out of state within the last 5 years and the facility did not process an out of state sex offender search or an out of state abuse hotline check. The Child Care Facility Handbook states on page 62 that: E. A copy of each request made to out of state child abuse and neglect registries for individuals who lived outside the state of Florida in the preceding five years. F. Background screening was not completed: [ after a 90-day break in service.] CCF Handbook, Section 5.2 Comments: There are currently employees. All 10 employee records were reviewed. One employee, CP, had an eligible background screening clearance on record, but there had been a 90 day break in service and a resubmission was not completed. The Director completed a resubmission in the Clearinghouse during the inspection. The employee is not allowed to be alone with children until a cleared live scan screening has been received. The Child Care Facility Handbook states on page 53 that: C. Child care personnel must be re-screened following a break in employment in the child care industry that exceeds 90 days. Child care personnel/individual with a break in service that exceeds 90 days are considered unscreened child care personnel/individuals until completion of re-screening. Another employee, SB, did not have one on file. Both employees signed the correct forms during the inspection. The Child Care Facility Handbook states on page 62 that: B. The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C Comments: There are currently 10 employees. Counselor reviewed 10 employee records. One employee, CD, had an expired Abuse/Neglect form. One employee, CP, started on 9/16/2024 and did not sign the form until 9/24/2024 and another employee, SB, was hired on 8/8/2024 and did not have the form on record. SB signed the form during the inspection. The Child Care Facility Handbook states on pages 61-62 that: C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before Child Care Facility Handbook Page | 62 hire date and annually thereafter by all child care personnel. 43. Enrollment Information CCF Handbook, Section 7.3 & s. 402.3054(2), F.S. (43-10)

The facility did not maintain documentation that the parent(s) or legal guardian(s) of each child were provided information regarding the potential for distracted adults to fail to drop off a child and leave them in the car annually during the months of April and September in that [9 newly enrolled children's parents were not notified in the month of September.]. CCF Handbook, Section 7.3, C.5. Comments: Nine newly enrolled children's records were reviewed since the last inspection on 6/10/2024. There was no documentation in the records to show that the children's parents were given the Distracted Adult notification information. The Child Care Facility Handbook states on page 61 that: 5. 33. Training Requirements CCF Handbook, Section 4 (33-09)

The facility did not have documented proof that all child care personnel were trained and knowledgeable within 30 days of date of hire in [ the use of fire extinguishers, safe sleep practices and shaken baby syndrome]. CCF Handbook, Section 4.2.3 and 4.2.5 Comments: There are currently 10 employees. All 10 employee records were reviewed. One employee, CP, has been at the facility for over 30 days and has not completed the fire extinguisher training. One employee, SB, has been at the facility for over 30 days and has not completed a course on Safe Sleep. The Child Care Handbook states on page 41 and page 42 that: 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. Documentation of training must be maintained on the Department's training transcript in the child care personnel record. 4.2.5 Fire Extinguisher Training All child care personnel shall be trained in the use and operation of a fire extinguisher, at each facility they are employed, within 30 days of date of hire.
