CCF Handbook, Section 4.7 Comments: The onsite interim Director does not have a required Directors Credential. Interim Director is in the process of completing and submitting information to obtain a Director Credential. 4.7 Director Credential Page 46 and 47 Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license. An applicant for the Director Credential must meet the requirements referenced in CF-FSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which is incorporated by reference in 65C-22.001(7)(k), F.A.C.    Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A Comments: A staff member was arguing and berating a 4-year-old in hallway. It was also noted that this staff member pushed child against wall and held her hand on his chest and yelled at child. These actions were observed by other staff, and no incident report was created or documented to inform parent of possible harm to child. A report has now been written and provided to the parent. The Child Care Facility Handbook states on page 55: 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. 40.    Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C Comments: A staff member was arguing and berating a 4-year-old in hallway. It was also noted that this staff member pushed child against wall and held her hand on his chest and yelled at child. These actions were observed by other staff, and no incident report was created or documented to inform parent of possible harm to child.It was reported that the child had also been pulled from underneath a table resulting in a rug burn. Staff member that inflicted possible physical and emotional harm resigned position at facility on 4/3/25. Specialist instructed provider to provide additional training on Child Abuse and Neglect Reporting to all current staff. The child care facility handbook states on page 63: 8.2 Child Safety . C. Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. Interactions with children that are aggressive, demeaning or intimidating in nature are strictly prohibited. Owner/Director/Staff Responsible Comments The School's Church Board held a meeting and gave the director the opportunity to resign her position which she did. The facility is now on a provisional license.
