   Complaint Related When staff are supervising children, they shall always ensure their care, protection, and guidance. On 11/11/2025, Child #1, (four years old), ran out of the classroom. Child #1 ran through two doors located inside of the building, and out of a door that led to outside. Video footage dated 11/11/2025 showed Child #1 run out of the entrance door and crossed the parking lot into the business parking lot that is next to the center. The main road in front of the center is approximately 88 feet from the entrance of the building, and the speed limit is 45 mph. Staff #3 retrieved the child and brought her back to the facility.    When staff are supervising children, they shall always ensure their care, protection, and guidance. On 10/20/2025 Staff #2, Staff #3 and Staff #4 were supervising 14 children on the playground. Based on review of video on 10/20/2025, Child #1 (age 2), was observed leaving the playground through an unlocked gate. Staff #2, Staff #3, and Staff #4 did not notice Child #1 leave the playground through the open gate. Staff #1 saw Child #1 outside of the playground and returned Child #1 to the playground. The gate out of the playground leads to a parking lot at the facility.
