Staff shall complete the Department?s health and safety update course annually. Documentation of the 2026 health and safety training for Staff #2 was overdue by 29 days, Staff #3 was overdue by 36 days, and Staff #4 was overdue by 11 days.

All staff who work directly with children shall have within 90 days of the date of employment or 90 days from subsidy vendor approval current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. Staff #4 did not have current certification in CPR and recertification was overdue by 10 days.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. A deck covering the sand box has broken and splintered floor board and railings. Floor boards from deck are broken and hanging down from the deck which poses a risk to children in care.

Annual approval from the health department shall be provided for meeting the requirements of food service. The center did not have a current Environmental Sanitation Inspection.

When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medication for Child A was onsite, but the medication authorization had expired over 22 days ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed, and the medication was not returned or disposed of as required.

The center shall maintain written records of children?s injuries that contain all of the required information. Three injury reports dated 03/06/2025, and 03/12/2025 did not document the date, time, and how parents were notified of the injury. Three injury reports were missing required signatures.

8VAC20-780-160-A

Based on records review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and shall have been completed within the last 30 calendar days of the date of employment. Evidence: The record for staff #1, date of employment 01/10/2024, contained a tuberculosis screening dated 02/27/2024. The tuberculosis screening was not conducted within 30 days prior to the date of employment.

Based on a review of five staff records, the center did not ensure a record was kept for one staff that contained the required information. Evidence: 1. The record of staff #1 (date of employment: 1/10/2024) identified the staff member as a lead teacher/program leader. The record was missing documentation to demonstrate that the individual possessed the certification/training required by the job position.
