The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan. The emergency plan was last reviewed by the provider on 3/2/24.

The family day home shall have a written emergency preparedness and response plan that addresses relocation. The emergency plan did not specify the relocation site. It stated "a nearby shelter, church or school."

The provider shall keep a record of prescription and nonprescription medication given children, which shall include the following: name of child, amount and type of medication administered, the day and time the medication was administered to the child, name of the caregiver administering the medication, any adverse reactions and any medication administration errors. There was no documentation of medication/supplement given to Child #9.

Medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication shall be kept in a locked place using a safe locking method that prevents access by children. Both medicinal items were not kept in a locked place. They were in diaper bags sitting on the floor near the food area. There was a safety gate across the area however, a child could have reached through the gate to the bags.

The family day home may administer nonprescription medication provided the medication is labeled with the child's name and given only at the dose, duration and method of administration specified on the manufacturer's label. Neither medicinal item was labeled with the child's name. While the provider said she had not given the medicine to Child #11; it is not known when or how much of the supplement was given to Child #9, as there was no medication administration record.

Prescription and nonprescription medications shall be given to a child according to the home's written medication policies and only with written authorization from the parent. Although the current medication policy signed by parents indicates that the provider will only give prescription medications, there were 2 non-prescription medication/supplement items on-site today. Additionally, there was no written authorization from the parents for these items. There was a medicinal product for gas, colic and hiccups on-site for Child #11; and a supplement for Child #9.
