Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1 and Child #6, enrolled for 2 weeks, was missing the following required components: names of persons authorized to pick up the child, name and phone number of child's physician, and the previous child day care attended by the child.
