Repeat Violation: There shall be at least 12 inches of space between occupied cots, beds, and rest mats. Exception: Twelve inches of space are not required where cots, beds, or rest mats are located adjacent to a wall or a divider as long as one side is open at all times to allow for passage. In the 2 year old classroom and Pre- K classroom there were 4 mats that were not 12 inches apart as required.

The center shall notify the parent by the end of the day of any known minor injuries. The center notification to parents did not have all required information. 1 out of 10 reports was missing future action to prevent reoccurrence. 1 out of 10 reports was missing time of incident.

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present. The allergy list was not posted in the Pre-K room.

Repeat Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. There was a shelf missing a wooden foot and was not stable, The red long rug protecting the carpet was bunched up creating a tripping hazard. Crack in drywall under window.

The diaper changing surface shall be clean and sanitized. The diaper changing pad was torn in many spots which prevented the pad to be cleaned and sanitized properly.

Areas and equipment of the center shall be maintained in a clean, safe and operable condition. The wood countertop accessible to children behind the diapering area was peeling off and there was peeling paint and a hole in the wall by the window.

Repeat Hazardous substances must be in a locked place that prevents access by children. In the two year old classroom, cleaning product were on the teacher counter within reach of children.

A ratio of 1 caregiver to 5 children is required for children ages 16 months up to 24 months. For approximately 15 minutes during the inspection there were 11 children being supervised by 2 caregivers.
