REPEAT VIOLATION: Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. On the playground, the plastic panel of the gate was broken in a circular shape with rough edges, large enough for a child to place their hand through. In the infant room, the floors of the restroom had an accumulation of dirt along the borders and buildup around the toilet, and the diaper changing pad in the restroom had a film of dirt within the crevices. In the two vans used for transporation, there were three car seats being used with various spots and dirt covering the seats.

On the playground at the bottom of the playground slide there was a tree root that was directly in front of the chute of the slide causing a tripping hazard. During the inspection on 11/14/2024 the Licensing Inspector observed a child coming off the slide and tripping over the tree root and falling.
