Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 03/13/2025, Child #1 (2 years old) was not within sight and sound supervision of staff. Staff #1 was left to care for Child #1 while Staff #2 took the remainder of class to the classroom. Staff #1 went to gym bathroom without communicating or transferring supervision to another staff member. Child #1 joined a line of students from another classroom leaving the gym and made it to the front doors of the center before exiting the building. Additional staff quickly followed after Child #1. Child #1 was out of sight for approximately one minute until being retrieved by a police officer passing by. Staff were able to catch up and retrieve the child from the police officer.

When staff are supervising children, they shall always ensure their care, protection, and guidance. On 03/13/2025 Staff #1 left a child unsupervised in the gym to use the restroom. No other staff was alerted to the teacher being indisposed. The child was able to exit the gym, leave through the front doors of the center, exited through an open gate in the parking lot, and into the road before staff were able to secure the child again. The opening in the parking lot gate is approximately 50 feet from the road where the child was found by a police officer.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. 1. The record for child #1 (enrolled for two months) was missing the parents work number. 2. The record for child # 3 (enrolled for two months) was missing the parent?s work contact number. 3. The record for child #4 and child #5, did not contain documentation of child updates and confirmation of up-to-date information.

A medication authorization, for a prescription medication, was observed for Child #6. The center did not have written authorization from the child?s physician and parent for a long-term medication.

(Repeat Violation) The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the required information. An injury report dated 10/01/2024, did not include the time of the incident, the date and time that the parent was notified, and the method in which the parent was notified.
