Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 4/14/26, staff #1 failed to stay within sight and sound supervision of the preschool class. She was in the bathroom changing one child, while the remaining 9 children in the class were not within her sight for a period of approximately 1 minute. The bathroom was within the classroom, but there was not a line of sight for the teacher to the rest of the class.

The diapering area shall have a nonabsorbent surface for diapering, and a leakproof storage system that is foot-operated so that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system. The diaper changing table pad in the older infant classroom was ripped so it was no longer nonabsorbent. The Toddler and two year old classrooms had foot-operated trash bins available, however diapers were found in trash bins that were not foot-operated
