The center is required to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child # 3 did not have documentation on file of immunizations at the time of the inspection.

Areas and equipment of the center, inside and outside, are required to be maintained in a clean, safe and operable condition. There were 2 broken plastic trucks with sharp edges inside of one of the open storage bins on the playground. There were also two large plastic cubes on the playground that were broken in multiple areas and had sharp edges. There was peeling white paint observed on the outside of one of the doors leading into the facility's school-age classrom from the playground.

The facility is required to have a working battery operated radio available in the event of an emergency. The battery operated radio at the facility was not in working order due to not having batteries.

Children under 10 years of age are required to always be within actual sight and sound supervision of staff. The Preschool classroom was transitioning the children from the playground back inside the classroom. At approximately 10:00am, Staff #1 did a face to name check and observed Child #1 walking into the Preschool classroom from the playground. Staff #1 then had the children use the restroom, wash hands and come to the circle for circle time. Staff #1 was asked by another child where Child #1 was. Staff #1 looked around and did not see Child #1. Staff #1 did a check of the restroom and then saw Child #1 outside of the classroom door on the playground. Staff #1 then opened the door and Child #1 came inside. Staff #2 was on the Two's playground and observed Child #1 by themself on the playground bench crying. As staff #2 was calling to Child #1 to come over to them, the Preschool door opened and the child went inside. Child #1 was without sight and sound supervision for approximately 2-3 minutes.

Child #3 only had documentation of the contact information for 1 of 2 required designated people to call in an emergency if a parent cannot be reached.

One of the classrooms was observed to be out of the required ratio from approximately 12:30pm-1:07pm. There were 11 children observed with 2 staff and the classroom required a 1:4 ratio according to the age of the children in care.

There was black pain on the metal fence on the playground that was peeling. In one of the classrooms, wood is broken/splintering on the sink cabinet.

Each staff member and individual from an independent contractor shall submit documentation of ta negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Staff #2 did not have a tuberculosis screening on file that was completed within the last 30 calendar days of their date of employment.
