The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The green chair in the Pre-K A classroom and the blue chair in the Preschool A classroom contained numerous tears in the seating material. In the two-year old classroom, peeling paint is present on the wall near the classroom entrance.

Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. An update for the information in the record of child A is overdue by 15 months.

Staff shall wash hands with soap and running water before and after a diaper change. Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff # 2 changed child A's diaper in the two year old classroom and did not wash the child's hands with soap and water. Staff #2 did not wash hands before or after changing child A's diaper and continued to assist child B with diapering.

The record for Child B, enrolled for 27 months, was missing one parent's work phone number. Child G, enrolled for 43 months, was missing both parent's work phone numbers. The record for Child E, enrolled for 45 days, was missing the address for one of two designated people to call in an emergency.

(Repeat violation) (Systemic deficiency) When staff are supervising children, they shall always ensure their care, protection, and guidance. On 9/11/2024, Child #1, 3 years old, exited the playground and entered an empty classroom to use the restroom without the awareness of any of the three staff, counted in the staff-to-children ratios, who were outside with children on the playground. Child #1 was later found inside the PreK A classroom by a parent, alone and crying. Child #1 was reportedly panicking because she was unable to open the door to exit.

(Repeat violation) (Systemic deficiency) Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 9/11/2024, Child #1, 3 years old, was out of sight and sound supervision for an estimated four to seven minutes. A parent found Child #1 inside the PreK A classroom, alone, and crying while the child?s class was outside on the playground.
