A tuberculosis (TB) screening is required every two years from the last screening. Staff #1 updated screening was due 9 days ago, and the updated screening has not yet been completed. Staff #2 updated screening was received 19 days late.

The center shall obtain documentation that each child has received required immunizations prior to the child's first date of attendance. The record for Child #5 (enrolled for 28 days) did not contain documentation of immunizations.

Each center shall maintain and keep at the center a separate record for each child, containing the name and phone number of the child's physician, and a name, address, and phone number of two emergency contacts. The record for Child #5 did not contain the name and phone number of the child's physician, and only had one emergency contact listed, with a name and phone number only.

At least every two years from the date of the prior tuberculosis screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Staff #3 had a TB screening that expired over 3 months ago.
