   The provider was out of compliance with R381-100-11(1)(f,g) for not ensuring each caregiver provides and maintains active supervision of each child by positioning themselves so each child in their assigned group is actively supervised and by not remaining aware of the entire group of children even when interacting with a smaller group or an individual child. During the investigation inspection, the evidence substantiated that a caregiver was not providing active supervision. During this time, two children were playing in an unsafe manner and one of the children was injured. Inspection: Investigation Inspection. Correction status: Manager approval, no follow up needed.
