In the outdoor area, there was a depression in the middle of the grassy area where water collects from the sprinklers, which was a little over two inches at the deepest mark, and greater than 5 by 5 inches in diameter at the time of the inspection. Correction action: On 11/14/2024, the licensor verified that the provider had no standing water accessible in the outdoor area, as stated in rule.
