A medication at the agency did not have the proper documentation and was not in the original box with the child's name. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: We will also make sure to have a medication form filled out with any medication brought in the building for a child before it is ever put in the medication cabinet that explains the exact way to administer it and how often. The medication will be sent home with the parent and let her know that it has to be in the original container- the box- with the child's medication in the correct name. To prevent the violation from happening again, we will make sure that every child has a medication form and the medication is in the original container.

Improper behavior management techniques were used at the agency. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Agency addressed educator, making her aware we cannot do that while being clocked in. She needs to be clocked out and out at her car if she has to discipline her own child. Make sure the employee does not do that again and understands it is a major violation. TDHS Child Care Licensing is requiring the educator complete training with Child Care Resource and Referal for the topics of Handle with Care and Developmentally Appropriate Practices

A supervision lapse occured at the agency on 04/06/2026. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The owner is readdressing bathroom procedures with the educator to ensure that there is a staff member available at the bathrooms when children use the restroom. Owner will address this with all of the classes. TDHS Child Care Licensing is requiring additional training provided by CCR&R for active supervision as well as training on transitioning out of the classroom.
