    A medication was observed to be expired with an expiration date of September 2025. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The agency will send the expired medication home with the family as soon as possible and will obtain family signatures on the medication form indicating that the medication was in fact return to the family. If a new medication is necessary, the agency will obtain a new medication and a new signed medication form granting permission for the medication to be given as necessary. The agency will also take measures to create and implement a plan for when and how medication expiration dates are checked and tracked on a monthly basis.
