A medication was observed to be expired with an expiration date of September 2025. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The agency will send the expired medication home with the family as soon as possible and will obtain family signatures on the medication form indicating that the medication was in fact return to the family. If a new medication is necessary, the agency will obtain a new medication and a new signed medication form granting permission for the medication to be given as necessary. The agency will also take measures to create and implement a plan for when and how medication expiration dates are checked and tracked on a monthly basis.

1240-04-01-.08 Record Keeping

A staff member hired on July 1, 2025, did not have a physical until 8-20-2025. The physical needed to be completed by 7-11-2025 to be in compliance. The corrective action plan was obtained from the agency and approved on 9/10/2025. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The agency plans to implement a no tolerance policy in which employees are expected to return the completed physical form on or before their tenth day of employment or they will not be scheduled to work from the eleventh day until it is returned. A record of this will be kept in the employee's file for licensing purposes.

The agency self-reported that a child in the Safari Scouts classroom ran away from their supervising teacher back into their classroom while the class was going to the restroom. — Visit: Incident or Injury Investigation,Safety Plan Served,Technical Assistance — Action taken: All agency staff will review Name to Face Roll Call procedures at the next staff meeting. The staff member was placed on a Safety Plan and will complete further training with Child Care Resource & Referral (CCR&R) in Active Supervision.
