An Epi-Pen was found in the agency that had an expiration date of 2/28/2026. — Visit: Safety Plan Served,Complaint Investigation,Technical Assistance — Action taken: Epi-Pen was immediately removed and returned to the parents at the end of the day. In the future agency will have the expiration date clearly posted with the new Epi Pen and will also check it monthly.

LC spoke with director about procedures to change license to cover age ranges from 1 year to 5 years old.

Classroom had children with mixed ages (17 months & 3 Years) that is not in compliance with DHS Licensing rules. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: Immediately, on Thursday, May 8th, we moved the 3 year old to the other classroom in the correct age group. We will be sure that each classroom is grouped correctly moving forward, according to the rules and regulations.
