During an unannounced visit on 6/1/26 it was determined after reviewing video footage that name to face roll call was not completed during transitions into the classroom from outside and a child wandered off. — Visit: Technical Assistance,Complaint Investigation,Incident or Injury Investigation,Safety Plan Served — Action taken: The agency corrected the violation with the following action(s): Teachers where written up and retrained on name to face. Name to Face sheets will be used going forward in addition to the ipads

During an unannounced visit on 6/1/26 it was determined after reviewing video footage that a child went into the play room unattended and was unsupervised for 20 minutes. — Visit: Technical Assistance,Complaint Investigation,Incident or Injury Investigation,Safety Plan Served — Action taken: The agency corrected the violation with the following action(s): the staff where written up and DHS was notified. Paper name to face sheets were implimented and staff where retrained on name to face.

During a visit on 2/2/26, video footage from 1/29/26 was observed and showed an educator in the 12-24 month classroom hitting a 20-month-old child on the hand with a toy after the child was fighting over the toy with another friend. — Visit: Complaint Investigation,Incident or Injury Investigation,Technical Assistance — Action taken: The agency corrected the violation with the following action(s): the educator was terminated and staff will be retrained

1240-04-01-.06 Staff

Two educators did not complete the required Licensure Rules training before having access to children. — Visit: QRIS Monitoring visit,Technical Assistance,Incident or Injury Investigation — Action taken: Teachers were required to immediately complete all required trainings. To prevent the violation from occurring again, the agency will create a list to refer back to for exactly which trainings are required prior to stepping into the classroom

Educator flicked a child in order to get her to be quiet during rest time. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: The teacher involved was immediately terminated, the family of the child involved was notified, and we self-reported to DHS. We are in the process of scheduling an all-staff training with CCR&R on positive reinforcement & appropriate behavioral management

Educator did not complete a safe sleep check every 15 minutes. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: The Educator immediately performed a safe sleep check on every child sleeping in the classroom. The agency implemented a binder with a tab section for each child & a weekly sheet in each section, split into days and 15 minute increments for teachers to mark whether child was sleeping on their back, side, or tummy. Also, discussed the importance of safe sleep checks and emphasized the importance of being diligent with setting the timer.
