The agency was not in compliance as documentation of physicals/health exams were not on file within ten calendar days of employment or work start date. — Visit: Complaint Investigation — Action taken: Both staff members have scheduled appointments and management plans to set reminders to ensusre all new hire physicals are on file before the 10 day period.

1240-04-01-.09 Incident Reporting

The agency was not in compliance as documentation was not provided to the parent/guardian the same day of incidents. — Visit: Complaint Investigation — Action taken: All documents will be signed by the director and parent the same day of the incident. The agency plans to have a designated location for documents to be turned in so they will not get misplaced.

1240-04-01-.11 Supervision

The agency was out of compliance with adult:child ratios during naptime in one of their classrooms. — Visit: Follow-up visit — Action taken: A second staff member was added to the classroom. Management plans to check daily the ages of the youngest child during naptime to ensure the agency is in compliance with adult:child ratio. All staff and management are scheduled to receive Supervision training by CCR&R.

1240-04-01-.12 Health and Safety

The agency was not in compliance as two educators' personal belongings were accessible to preschool age children. — Visit: QRIS Monitoring visit — Action taken: Staffs' personal belongings were moved to where they were inaccessible to the childen. Educators were instructed to keep personal belongings out of the children's reach. A designated place for staffs' personal belongings was discussed.

1240-04-01-.11 Supervision

The agency was not in compliance with the playground supervision plan as roll call was not conducted upon the children returning to their classroom. — Visit: QRIS Monitoring visit — Action taken: Management informed educators that roll call has to be conducted before leaving the classroom, upon entering the playground, before leaving the playground, and upon returning to class. All educators and management will be required to receive Supervision training from CCR&R.

Children's hands were not washed after breakfast in a toddler classroom. — Visit: QRIS Monitoring visit — Action taken: Educators were re-trained on proper handwashing procedures.

1240-04-01-.14 Equipment for Children

Electrical cords were accessible to children in the 4 yrs. - 5 yrs. old class. — Visit: QRIS Monitoring visit — Action taken: Administrative staff will conduct regular walk throughs to make sure there are no electrical cords accessible to children.
